Clinical Care Manager – Aged Care (Home Care Packages)

The Opportunity
Unite Healthcare is partnering with a well-established and growing community aged care provider to recruit an experienced Clinical Care Manager. This role is ideal for a senior nurse or care manager with strong Home Care Package (HCP) experience who is passionate about delivering high-quality, client-centred care in the community.
You will play a key role in overseeing clinical care delivery, managing complex client needs, and ensuring services align with the Aged Care Quality Standards and evolving aged care reforms.
Key Responsibilities
Clinical & Care Management
- Manage a caseload of Home Care Package and community care clients
- Conduct comprehensive client assessments, reviews and risk management
- Develop, implement and evaluate individualised care plans
- Coordinate clinical and non-clinical supports to meet changing client needs
- Support transitions of care as client circumstances evolve
Leadership & Operations
- Provide clinical leadership and guidance to care teams
- Ensure services are delivered in line with policies, procedures and quality standards
- Monitor care budgets and support effective service utilisation
- Collaborate with internal teams and external providers to ensure continuity of care
Compliance & Quality
- Maintain compliance with Aged Care Quality Standards and relevant legislation
- Contribute to audits, quality improvement initiatives and clinical governance
- Support organisational readiness for aged care reforms, including Support at Home
Stakeholder Engagement
- Build strong relationships with clients, families and referrers
- Act as a trusted clinical point of contact for complex care matters
About You
Essential Criteria
- Registered Nurse or Enrolled Nurse with current AHPRA registration
- Proven experience as a Care Manager / Clinical Care Manager within Home Care Packages
- Strong knowledge of aged care legislation, funding models and quality standards
- Excellent communication, documentation and organisational skills
- Ability to manage complex clinical situations with professionalism and empathy
- Current Australian driver’s licence
